Referral Partnership Agreement

Purpose

This agreement is between Christmon Housing Solutions LLC ("CHS") and the referring agency ("Partner"). Its purpose is to establish a collaborative referral relationship supporting individuals transitioning from shelter, eviction, or unsafe housing into stable, structured independent living.

Agreement

• Partner agrees to refer eligible individuals who meet Christmon Housing Solutions criteria.
• Christmon Housing Solutions agrees to review all referrals in a timely, respectful manner.
• This agreement does not create a legal obligation for housing placement.
• Both parties will maintain confidentiality and professional communication.
• Both parties agree to act in good faith to support successful placements.
• Christmon Housing Solutions does not disclose the location of its residences. Partner agrees not to request or publish property addresses.
